Whats the Difference?

Our logo is not just cute. It signifies the professional support built into a program to deliver results. This is done with you the physician, a health coach, a dietitian and a therapist. Hands-On Weight Management is a year long program that teaches all the back-to-basics necessities of healthy living, in an environment of direct professional support, with a duration to build and sustain life-long change.

 

Whereas most other weight management discussions happen either in a clinical setting or on zoom, we work directly with the client in their worlds... grocery shopping with them where they normally shop, cooking with them in a community kitchen, and walking with them for exercise while having coaching conversations.  This enables them to develop a new normal since it is in their normal worlds and not abstract.

 

This comprehensive and multi-faceted hands-on approach is designed to educate and deliver lasting behavioural change that actually empowers the client; not leaving them bewildered as many new programs/diets do. Because we address psychological, behavioural, nutritional and physical activity factors by using kinesthetic learning over a long period of time, Hands-On Weight Management offers a highly effective support system that will deliver sustainable weight loss.

 

Clients are working one-on-one with a coach learning a new way of living over time. They learn how to grocery shop in a more health conscious way and how to cook these high-protein, high-fibre meals that have been custom built for them by a dietitian. They have accountability in getting exercise by meeting the coach/partner for their walks. Clients are getting emotional support from the coach and social support from other program participants as they are developing new habits and mindsets.

 

By coming up with their own goals and envisioning their futures differently in coaching conversations, clients discover their own intrinsic motivations and how to achieve them. Self-determination theory and intentional change theory are the basis for this coaching. The client gains autonomy in the process and no longer feels like life is happening to them, but they start to take control of their own lives. They gain competency in how to live healthier lives and become more connected to a community of others who are on the same journey.

 

We are not focused on the numbers on the scale, but laser-focused on the mindset and lifestyle changes that will produce the long-term results that are being sought. That does not mean that we aren’t measuring progress though - we expect for clients to have monthly check-ins with you to monitor it.  We collaborate with you- the referring health care practitioner, a dietician to deliver a customized meal plan to meet their specific dietary needs, and a therapist who can get to the root of some of the issues. We don’t want to just put a band-aid over the problem - we want to help your patients build intentional lives that they can be proud of. We are here to be hands-on with your patients in the way you would want to be with them, but you just can’t, given the time restraints of your practice.

Program Highlights

Total Duration: One Year

 

Intensive Phase:

 

12 Weeks Four supportive coaching sessions each week PLUS appointments with dietitian and therapist

 

Food related - one grocery shopping outing, one community cooking session

 

Exercise related - two one-on-one walking coaching sessions each week

 

Intermediate Phase:

 

24 Weeks Two supportive coaching sessions each week PLUS appointments with therapist

 

Food related - one community cooking session

 

Exercise related- one partnered exercise walking session with coach, but encouraged to meet with another program participant to maintain two exercise sessions/week

 

Independent Phase:

 

12 Weeks One supportive coaching session each week PLUS one final appointment with therapist

 

Alternating weekly- one community cooking session or one partnered exercise session with coach
